VIETNAMESE PASSPORT APPLICATION PROCEDURES

1. YOU MUST KNOW BEFORE APPLYING FOR A VIETNAMESE PASSPORT:

1.1. You can apply for a Vietnamese passport only if you can prove to be Vietnamese citizen.

1.2. You should be present at the Embassy if you apply for a new Vietnamese passport.

1.3. You should fill out the application form in Vietnamese yourself (Parents can fill out the form on behalf for children).

1.4. You have to sign and keep your passport carefully because it's a national property.

1.5. You can submit the application at the Embassy or send it by mail to the Embassy. In case you apply by mail, mailing address and telephone/cellphone number will be required for the documents to be shipped back by Fedex.

1.6. Passports are valid for 10 years (5 years if the passport holder is under 14).

1.7. Processing time: 

- For passport renewal and new passport application for children: 5-7 business days upon the receipt of the complete dossiers.

- For lost/damaged passport application: 10-15 business days upon the receipt of the complete dossiers.

- For new passport application for adults: 30-45 business days upon the receipt of the complete dossiers.

2. NEW PASSPORT APPLICATION:

2.1. For children

Application form (in Vietnamese - Please refer to the below instructions)

- Original Vietnamese birth certificate (or a notarized and authenticated copy)

- Notarized and authenticated copy of their Canadian resident document (passport/PR card/Citizenship) (Click HERE to see a sample of authenticated notarized copy of the passport).

- 02 new passport sized photos

- For the Canadian name to be printed on the new passport, it is required to provide a copy of the Certificate of Name change.

2.2. For adults

Application form (in Vietnamese - Please refer to the below instructions)

C. V (Biography form in Vietnamese)

- Original Vietnamese birth certificate (or a notarized and authenticated copy)

- Notarized and authenticated copy of their Canadian resident document (passport/PR card/Citizenship) (Click HERE to see a sample of authenticated notarized copy of the passport).

- 02 new passport sized photos

- For the Canadian name to be printed on the new passport, it is required to provide a copy of the Certificate of Name change.

3. PASSPORT RENEWAL APPLICATION DOSSIERS:

3.1. Application form (in Vietnamese - Please refer to the below instructions)

3.2. Original Vietnamese passport (to-be-expired within 6 months)

3.3. Notarized and authenticated copy of their Canadian passport/PR card/Citizenship card (Click HERE to see a sample of authenticated notarized copy of the passport).

3.4. 02 new passport sized photos

3.5. A notarized copy of the Vietnamese birth certificate (if the Vietnamese passport is expired)

3.6. For the Canadian name to be printed on the new passport, it is required to provide a copy of the Certificate of Name change.

4. LOST/DAMAGED PASSPORT APPLICATION

Application form (in Vietnamese - Please refer to the below instructions)

- Original passort (in case of damage)

- Vietnamese documents (birth certificate, ID card or passport or notarized and authenticated copy of the document)

- Police report

Embassy report (in Vietnamese)

- Notarized and authenticated copy of their Canadian resident document (passport/PR card/Citizenship, work permit, study permit) (Click HERE to see a sample of authenticated notarized copy of the passport).

- 02 new passport sized photos

- For the Canadian name to be printed on the new passport, it is required to provide a copy of the Certificate of Name change.

*INSTRUCTIONS FOR FILLING IN THE APPLICATION FORM FOR PASSPORT RENEWAL:

Number 1: FULL NAME (ORIGINAL VIETNAMESE NAME)

Number 2: GENDER

Number 3: DATE OF BIRTH

Number 4: PLACE OF BIRTH

Number 5: VIETNAMESE ID CARD INFORMATION (IF ANY)

Number 6, 7: ADDRESS IN THE CANADA, PHONE AND EMAIL

Number 8: ADDRESS IN VIETNAM BEFORE THE CHILD LEAVE FOR CANADA

Number 9: N/A.

Number 10: FATHER’S NAME, MOTHER’S NAME (ADOPTIVE PARENTS)

Number 11: LATEST VIETNAMESE PASSPORT NUMBER, ISSUED ON...BY....

Number 12: REQUEST TO RENEW EXPIRED OR EXPIRING PASSPORT

Number 13: N/A.

Number 14: DOCUMENTS SUBMITTED.

 

**Parents can sign the form on behalf of their child. It is not requested to fill in the remaining of the form. In case you want to have your child's new Canadian name in the new passpsort, we can print the new name on page 6 or 7 of the new Vietnamese passport. You need to provide a copy of the certificate of name change (or any paper showing that the Vietnamese name and the Canadian one belong to the same person).

5. FEES

5.1. Passport application fee (including new passport and renewal): USD 70.00 e.a

5.1. Lost/damaged passport application fee: USD 150.00 e.a

All the fees can be paid by cash, money order or credit card (Visa or Master card only).

*Note

- The applicants may request for the supporting services for notarization and authentication. 

- For further information, please kindly contact the Passport section at 613 236 0772 (ext 228). 

- The walk-in hours of the Consular services:

Monday: 01.30 - 04.30 pm

Wednesday and Friday: 09.30 am - 12.30 pm;  01.30 - 04.30pm.

- If you request to have the documents returned by mail, please provide full name of the recipient, a detailed mailing address (with postal code) and a telephone/cellphone number. The documents will be shipped by Fedex service (next day delivery, fee: USD 35.00/package) or Canada post service (2-3 business days, USD 25.00/package). 

Due to the delayed delivery of Canada Post, Fedex service is recommended for urgent document shipping.
