Cremated Remains/Dead Body to Vietnam

PROCEDURES FOR SHIPPING CREMATED REMAINS/DEAD BODY TO VIETNAM

1. SUBJECTS:

  • Cremated remains/Dead body of the deceased Vietnamese;

  • Cremated remains/Dead body of the deceased Vietnamese-Canadian

2. DOSSIERS:

2.1. For Cremated remains:

  • 01 completed application (Download Form PDF   05/NG-LS in Vietnamese). (Please complete all the required fields, especially the information of place of landing, name of carrier, flight number, and the arrival time to Vietnam);

  • 01 original or certified copy of the applicant’s personal documents (such as passport, ID card/citizenship card…);

  • 01 original or certified copy of Certificate of death;

  • 01 original or certified copy of Certificate of cremation;

2.2. For Dead body:

  • 01 completed application (Download Form PDF  05/NG-LS in Vietnamese). (Please complete all the required fields, especially the information of place of landing, name of carrier, flight number, and the arrival time to Vietnam);

  • 01 original or certified copy of the applicant’s personal documents (such as passport, ID card/citizenship card…);

  • 01 original or certified copy of local Certificate of quarantine control issued by Canadian authorities;

  • 01 original or certified copy of Certificate for shipment of dead body out of province/Canada issued by Canadian authorities;

  • 01 copy Certificate confirms that nothing but the body in the casket issued by Canadian authorities;

  • 01 scan/copy of a permission of Vietnamese authority in the province (where the body will be buried);

  • 01 copy of the deceased person’s personal documents that prove he/she originated from Vietnam (birth certificate, ID card…)

3. FEES:

  • Fees must be paid in form of US$ cash, US$ money order or US$ bank draft, payable to “The Embassy of Vietnam” or credit card (Please be aware that 3% of total amount will be automatically charged for credit card transaction fee).   • If you request to have the documents returned by mail, please provide full name of the recipient, a detailed mailing address (with postal code) and a telephone/cellphone number. The documents will be shipped by Fedex service or Canada post service. Additional charge may be applied for remote areas shipping. Without the mailing service specification, your documents will be returned by Fedex.

  • The normal processing time is five (05) to seven (07) business days since the receipt of complete application.

4. NOTES:

  • Conditions for shipping dead body/cremated remains:

    • The cause of dead does not come from transmitted diseases;

    • Dead body must be laid in a coffin with zinc protecting cover and lined with PVC sheets and absorbent materials; outward wooden trunk, sealed with sealing wax by security and health authorities;

    • Dead body must be wrapped by 2 PVC sheets and put into a cloth bag and outward porcelain box;

    • Cremated remains must be in a tightly closed container.

  • Shipping of dead body caused by transmitted diseases is prohibited.
